CHANGE OF NAME IN THE PASSPORT

Check list for the documents required

  • Print out of duly filled online Indian Passport Application Form
  • Duly filled in Miscellaneous Services Application Form
  • Two passport-size recent color photographs
  • Current passport in original and copies (first & last page)
  • Copy of Residence Permit/Work permit (front & back side)
  • Original newspaper announcement announcing change of name, published in two leading daily newspapers. One newspaper should be of area of applicant's permanent address in India and the other of the area of his present address in Sweden/Latvia. Publishing should be in English. E-paper announcement aren’t acceptable.
  • Affidavit in prescribed format signed in presence of Consular Officer which can be found under Application forms.
  • Latest Personbevis issued by Skatteverket for residents of Sweden
  • One local address proof for residents of Latvia
  • A self -addressed stamped REK envelope- if you want the passport to be posted back to you (Applicants living in Latvia may send self-addressed envelope with 11 International Reply Coupons)
  • Proof of payment of SEK 639/- (for 36-page booklet) or SEK 846/- (for 60-page booklet)
  

CHANGE OF NAME FOLLOWING A MARRIAGE

Upon marriage, if the Indian women citizens wish to change their names (change of given name or change/addition of surname), they can do so by following a procedure of submitting the following documents with the Embassy:
 

Check list for the documents required:

  • Print out of duly filled online Indian Passport Application Form
  • Duly filled in Miscellaneous Services Application Form.
  • Two passport-size recent color photographs
  • Current passport in original and copies (First & last page)
  • Copy of Residence Permit/Work permit (front & back side)
  • Marriage certificate issued by Registrar of Marriage (attested by the Home Department of concerned State Government and then by the Consular Division in the Ministry of External Affairs, New Delhi). If issued in Sweden/Latvia, the marriage certificate should be duly notarized. Marriage certificate in Latvian should be translated in English.
  • A joint affidavit signed by the couple along with a joint photograph, to be attested by the Embassy. (Please check Applications forms for related Affidavit)
  • Latest Personbevis issued by Skatteverket for residents of Sweden
  • One local address proof for residents of Latvia
  • REK envelope, if you want the passport to be posted back to you.
  • Applicants living in Latvia may send self-addressed envelope with 11 International Reply Coupons
  • Proof of payment of SEK 639/- (for 36-page booklet) or SEK 846/- (for 60-page booklet)
PLEASE NOTE: Attestation of affidavit would be taken care of by the Attestation Section. A separate appointment with Attestation section needs to be booked for the same. Passport application can be submitted only after the procedure for attestation for respective affidavit is fulfilled.
